If you answered YES:

For Non-Profit/Community Education Partners:

  1. Country of Registration: Select the country where your organization is legally registered.
  2. Charitable Registration Number: Provide your official registration number (e.g., “123456-AB” for a UK charity).

For All Other Entity Types (Corporate Partner, Educational Institution, etc.):

If you answered NO:

  • Reason for Non-Registration: Briefly explain why your entity is not registered (e.g., “We are a newly formed collective awaiting formal registration”).

Question 3: Official Name of Your Entity

Provide the legal name of your organization or your full name (for individuals).

Primary Representative Details

The primary representative is the official contact responsible for the entity’s application.

Questions 1–4: Name Details

  1. Prefix: Select appropriate title (e.g., Dr., Prof., Ms.).
  2. First Name: Legal first name.
  3. Middle Name: Optional (if applicable).
  4. Last Name: Legal surname.

Question 5: Primary Representative’s Role

  • Describe their position and relevance to the entity.

Examples:

  • Grassroots Group: “Co-Founder & Program Director”
  • Corporate Partner: “Head of Corporate Social Responsibility”

Questions 6–7: Contact Information

  • Phone Number: Provide a working number with country code (e.g., +1-555-123-4567).
  • Email Address: Use an official/professional email (e.g., name@university.edu).

Questions 8–9: Professional & Social Media Links

  • LinkedIn Profile: Mandatory. Share a full URL (e.g., linkedin.com/in/johndoe).
  • Social Media:

Example:
